A Proposed Solution

Jianshn.com was created to address this problem of access and health inequity experienced by patients suffering from musculoskeletal pain.

  • Reducing Barriers: After diagnosis from physician, patients are provided a hard-copy, generic home exercise program with to high-quality exercise videos with instruction. This reduces wait-times and cost-barriers to see a physiotherapist.

  • Emphasis on Safety: Equipment-free range of motion, isometrics and isotonic exercises created by a physiotherapist and rehabilitation expert to decrease risk of additional injury or other exercise contraindications.

  • Health Equity and Privacy: Free for patients to use with no specific personal identifying information needed to access the exercises.

Even with the proven health benefits of exercise for musculoskeletal pain, a stark disconnect remains in Canadian healthcare. A 2018 study in the British Columbian Medical Journal found that:

  • Low Activity & Prescriptions: Only 17.6% of Canadians meet physical activity guidelines, yet just 15.8% of physicians provide exercise programs.

  • Physician Barriers: Practicing physicians frequently cite lack of time and remuneration.

Upon payment (monthly subscription) the physician will be provided access to the exercise handouts for different body areas of musculoskeletal problems such as the neck, shoulder, low back, and knee. Once the patient is assessed and is appropriate to perform exercises at home. The handouts can be printed off during a consultation for the patient to use at home.

Additionally, a 2017 study found that publicly funded primary care teams in Canada rarely incorporate physiotherapists, creating a significant bottleneck in patient care:

  • Disproportionate Workforce Distribution: Only 3% of Canadian physiotherapists worked in community health centers, compared to 46% in hospitals/residential care and 20% in solo practices.

  • Provider Dissatisfaction: 79% of Canadian family physicians report unsatisfactory access to publicly funded physiotherapists.

  • Patient Barriers: Patients long wait-lists or no access to public outpatient physiotherapy clinics to help with neck, shoulder or low back pain.
